Human Excretory System – Kidneys, Nephron, Urine Formation & Dialysis

🎯 Key Points /
1
Kidneys are the primary excretory organs of the human body. They are bean-shaped, reddish-brown, retroperitoneal organs lying on either side of the vertebral column between the levels of the 12th thoracic and 3rd lumbar vertebrae.
2
Each kidney is about 10 to 12 cm long, 5 to 7 cm wide, 2 to 3 cm thick and weighs roughly 120 to 170 grams. The right kidney lies slightly lower than the left because the liver occupies the space above it.
3
The nephron is the structural and functional unit of the kidney. Each kidney contains about one million nephrons, giving nearly two million in the body.
4
A nephron has a Bowman’s capsule with its glomerulus, a proximal convoluted tubule (PCT), a loop of Henle, a distal convoluted tubule (DCT) and a collecting duct. The glomerulus together with Bowman’s capsule is called the Malpighian corpuscle or renal corpuscle.
5
The kidneys receive about 20 to 25 per cent of the cardiac output, which is close to one litre of blood every minute. This heavy blood supply is what makes such rapid filtration possible.
6
Urine formation takes place in three steps: glomerular filtration (ultrafiltration), selective reabsorption and tubular secretion.
7
The glomerular filtration rate (GFR) of a healthy adult is about 125 ml per minute, which works out to nearly 180 litres of filtrate a day. Yet only 1 to 1.5 litres of urine is passed, because about 99 per cent of the filtrate is reabsorbed.
8
The proximal convoluted tubule reabsorbs nearly 70 to 80 per cent of the filtered water and electrolytes and the whole of the filtered glucose and amino acids. Glucose appearing in urine (glycosuria) therefore points to a problem such as diabetes mellitus.
9
The counter-current mechanism, operating between the loop of Henle and the vasa recta, maintains the osmotic gradient of the medulla and allows human urine to be concentrated up to about four times the osmolarity of blood plasma.
10
The juxtaglomerular apparatus (JGA) secretes renin, which starts the renin-angiotensin-aldosterone system (RAAS). Angiotensin II constricts blood vessels and makes the adrenal cortex release aldosterone, which increases sodium and water reabsorption and so raises blood pressure.
11
Antidiuretic hormone (ADH or vasopressin), released from the posterior pituitary, increases water reabsorption in the DCT and collecting duct. Its deficiency causes diabetes insipidus, marked by the passing of large volumes of dilute urine.
12
Atrial natriuretic factor (ANF), released by the atria of the heart when blood volume rises, dilates blood vessels and lowers blood pressure. It therefore acts as a natural check on the RAAS.
13
Humans are ureotelic: the ammonia produced by protein breakdown is converted into the less toxic urea in the liver through the ornithine or urea cycle. Bony fishes and aquatic amphibians are ammonotelic, while reptiles, birds, land snails and insects are uricotelic.
14
The kidney is not only an excretory organ. It also secretes the hormones erythropoietin and renin and converts vitamin D into its active form, calcitriol. Besides the kidney, the lungs give out carbon dioxide and water vapour, the sweat and sebaceous glands of the skin remove salts, urea and lactic acid, and the liver eliminates bile pigments and cholesterol.
15
In kidney failure, haemodialysis is carried out with an artificial kidney in which blood flows past a cellophane semipermeable membrane; heparin is added to prevent clotting and protamine is given afterwards to restore it. Renal calculi are mostly calcium oxalate stones, and World Kidney Day is observed on the second Thursday of March.
📖 Detailed Notes – English

Why the body must excrete

Every living cell breaks down proteins, nucleic acids and other nitrogen-bearing molecules, and the ammonia released in the process is extremely toxic even in small amounts. The excretory system exists to clear these nitrogenous wastes while simultaneously guarding the volume, composition, pH and osmolarity of the body fluids. This second duty is called osmoregulation, and it is the reason the kidney is studied as a regulatory organ rather than a mere drain. In human beings ammonia is converted in the liver into the far less toxic urea through the ornithine cycle, also called the urea cycle, and because urea is our principal nitrogenous waste we are classed as ureotelic animals. Bony fishes and aquatic amphibians, surrounded by water they can spare, flush ammonia out directly and are ammonotelic. Reptiles, birds, land snails and insects must hoard every drop of water, so they convert waste into almost insoluble uric acid and are uricotelic. The pattern is simple: the less water an animal can afford to lose, the drier and less toxic its nitrogenous waste.

Organs of the human excretory system

The system consists of a pair of kidneys, a pair of ureters, a urinary bladder and a urethra. The kidneys lie retroperitoneally, that is behind the abdominal lining, on either side of the vertebral column between the levels of the twelfth thoracic and third lumbar vertebrae. Each is about 10 to 12 cm long, 5 to 7 cm wide and 2 to 3 cm thick, and weighs roughly 120 to 170 grams. Because the liver takes up the space above it, the right kidney sits a little lower than the left – a favourite one-mark question. On the inner concave border is a notch called the hilum, through which the renal artery enters and the renal vein and ureter leave. A longitudinal section shows an outer renal cortex and an inner renal medulla containing eight to eighteen conical renal pyramids. Cortical tissue dipping between the pyramids forms the renal columns, or columns of Bertini. Urine drains from the pyramids into the calyces, then the renal pelvis, and on into the ureter.

The nephron

The nephron is the structural and functional unit of the kidney, and each kidney holds about one million of them. At its head is the cup-shaped Bowman’s capsule, which encloses a dense knot of capillaries called the glomerulus. Blood reaches the glomerulus through a wide afferent arteriole and leaves through a narrower efferent arteriole; this difference in calibre is what keeps glomerular pressure high. Bowman’s capsule together with the glomerulus forms the Malpighian corpuscle, or renal corpuscle. From the capsule the tubule runs as the proximal convoluted tubule, dips as the loop of Henle with its descending and ascending limbs, returns as the distal convoluted tubule and finally joins a collecting duct. Roughly 85 per cent of nephrons are cortical, with short loops; the remaining 15 per cent are medullary nephrons whose long loops plunge deep into the medulla alongside a special capillary bed, the vasa recta.

Urine formation in three steps

1. Glomerular filtration. Driven purely by blood pressure, this is a physical process also called ultrafiltration. Blood cells and most plasma proteins are held back, while water, glucose, amino acids, salts, urea and other small molecules pass into the capsule. The kidneys receive 20 to 25 per cent of the cardiac output, close to a litre of blood a minute, and the resulting glomerular filtration rate is about 125 ml per minute, or nearly 180 litres a day. The net filtration pressure is only about 10 mm Hg, because the glomerular hydrostatic pressure of roughly 55 mm Hg is opposed by a capsular pressure of about 15 mm Hg and a blood colloid osmotic pressure of about 30 mm Hg.

2. Selective reabsorption. Of those 180 litres, about 99 per cent is taken back, which is why only 1 to 1.5 litres leaves the body as urine. The proximal convoluted tubule, with its brush border of microvilli, reclaims 70 to 80 per cent of the water and electrolytes and the whole of the filtered glucose and amino acids. Sugar in the urine, or glycosuria, therefore signals that the reabsorptive capacity has been exceeded, as in diabetes mellitus. The descending limb of Henle’s loop is permeable to water but not to salts; the thick ascending limb is the reverse, actively pumping sodium chloride out while staying impermeable to water. The distal convoluted tubule and collecting duct make the final, hormone-controlled adjustments.

3. Tubular secretion. Hydrogen ions, potassium ions, ammonia, creatinine and many drugs are actively added from the blood into the filtrate. This step is what allows the kidney to hold blood pH near 7.4 and to clear substances that filtration alone would leave behind.

The counter-current mechanism

The loop of Henle and the vasa recta lie side by side with their fluids flowing in opposite directions, and this arrangement multiplies a small difference in concentration into a steep gradient. Sodium chloride pumped out of the ascending limb raises the osmolarity of the medullary interstitium, which in turn draws water out of the descending limb and later out of the collecting duct. Urea recycled from the collecting duct deepens the gradient further. Osmolarity climbs from about 300 milliosmoles per litre in the cortex to roughly 1200 at the tip of the medulla, so human urine can be concentrated to about four times the strength of blood plasma. The sluggish flow of blood through the vasa recta is essential, since faster flow would wash the gradient away.

Hormonal regulation

The juxtaglomerular apparatus, formed by modified cells of the afferent arteriole together with the macula densa of the distal tubule, releases renin whenever renal blood flow or blood pressure falls. Renin acts on plasma angiotensinogen to make angiotensin I, which the angiotensin-converting enzyme changes into angiotensin II. Angiotensin II is a powerful vasoconstrictor that raises glomerular filtration and also prompts the adrenal cortex to release aldosterone, which steps up sodium and water reabsorption in the distal tubule. This whole chain is the renin-angiotensin-aldosterone system, or RAAS. Working alongside it, antidiuretic hormone – made in the hypothalamus and released from the posterior pituitary – increases facultative water reabsorption when the body is dehydrated; its deficiency causes diabetes insipidus, in which large volumes of dilute urine are passed. Pulling the other way, atrial natriuretic factor from the heart atria dilates vessels and lowers blood pressure when blood volume rises, acting as a built-in brake on the RAAS.

Micturition and the composition of urine

Urine collects in the urinary bladder until stretch receptors in its wall report a volume of roughly 200 to 300 ml to the central nervous system. The micturition reflex then contracts the detrusor muscle and relaxes the sphincters, and urine is expelled through the urethra. An adult passes about 1 to 1.5 litres a day. Urine is roughly 95 per cent water and is slightly acidic at about pH 6; urea is its chief solute, with 25 to 30 grams excreted daily, accompanied by creatinine, uric acid, sodium chloride and potassium. This is why urine analysis is a standard diagnostic tool – glucose suggests diabetes mellitus, ketone bodies suggest uncontrolled fat breakdown, and albumin or blood points to damage of the filtering membrane itself.

Accessory excretory organs, disorders and dialysis

The kidney does not work alone. The lungs breathe out carbon dioxide and a good deal of water vapour, the sweat and sebaceous glands of the skin remove sodium chloride, urea, lactic acid and sterols, and the liver disposes of bile pigments, cholesterol, degraded steroid hormones, some vitamins and many drugs. The kidney in turn is itself an endocrine gland, secreting erythropoietin to drive red blood cell production, renin for pressure control, and converting vitamin D into its active form calcitriol. Common disorders include uraemia, the accumulation of urea in blood; glomerulonephritis, an inflammation of the glomeruli; and renal calculi, the stones that are most often calcium oxalate. When the kidneys fail, haemodialysis is performed with an artificial kidney in which blood is drawn past a cellophane semipermeable membrane bathed in a dialysing fluid of the same composition as plasma but free of nitrogenous wastes, so that those wastes diffuse out while useful solutes stay in. Heparin is added to stop clotting during the procedure and protamine is given afterwards to restore normal coagulation. The permanent remedy is kidney transplantation, which in India is governed by the Transplantation of Human Organs Act, 1994, amended in 2011 to also cover tissues. World Kidney Day is observed on the second Thursday of March.

💡 Memory Tips /
FRS – the three steps in order: Filtration, Reabsorption, Secretion. Remember it as Filter First, Reclaim Rapidly, Secrete Surplus.
Nephron road map – ‘Big Pots Hold Dry Coffee’: Bowman’s capsule to PCT to loop of Henle to DCT to Collecting duct. The filtrate always travels in that order.
The 125 – 180 – 1.5 ladder: 125 ml a minute filtered, 180 litres a day of filtrate, only 1.5 litres a day of urine. Each number drops one step, and the gap between 180 and 1.5 is the 99 per cent that is reabsorbed.
Waste by habitat – ‘Water, Us, Up’: Ammonia for animals in water, Urea for us mammals, Uric acid for creatures that go up in the air like birds. Less water available means a drier waste.
ADH versus aldosterone: ADH brings back the Water (think A-D-H: Add Dihydrogen… H2O); Aldosterone brings back the Salt (think Aldosterone – sodium Also). ANF is the opposite of both and lowers blood pressure.
