🫁 Human Respiratory System — Breathing and Exchange of Gases

🎯 Key Points  |

1

The human respiratory system comprises the external nostrils, nasal chamber, pharynx, larynx, trachea, two bronchi, bronchioles and the alveoli, where actual gas exchange takes place.

2

The larynx is called the sound box or voice box; the epiglottis is a cartilaginous flap that covers the glottis during swallowing and prevents food from entering the trachea.

3

The trachea is about 11 cm long and divides at the level of the 5th thoracic vertebra into the right and left primary bronchi. C-shaped cartilaginous rings keep it from collapsing.

4

The right lung has three lobes and the left lung has only two lobes, because space on the left side is occupied by the heart.

5

Each lung is enclosed in a double-layered pleura. The pleural fluid between the two layers reduces friction on the lung surface during breathing.

6

The two lungs together contain about 300 million alveoli, providing a respiratory surface area of roughly 70 to 80 square metres — nearly 40 times the body surface area.

7

Inspiration: the diaphragm contracts and flattens and the external intercostal muscles raise the ribs and sternum, increasing thoracic volume, so intra-pulmonary pressure falls below atmospheric pressure and air rushes in.

8

Normal expiration is a passive process — the diaphragm and intercostal muscles relax, thoracic volume decreases and intra-pulmonary pressure rises above atmospheric pressure.

9

A healthy adult breathes 12 to 16 times per minute. With a tidal volume of about 500 mL, pulmonary ventilation is roughly 6000 to 8000 mL per minute.

10

Vital Capacity = IRV + TV + ERV, about 4000 to 4800 mL. Total Lung Capacity = Vital Capacity + Residual Volume, about 5800 to 6000 mL.

11

Residual Volume is about 1100 to 1200 mL of air that can never be forced out of the lungs, and therefore it cannot be measured directly by a spirometer.

12

Oxygen transport: about 97% of oxygen is carried by red blood cells as oxyhaemoglobin and only about 3% is dissolved in plasma. One haemoglobin molecule binds four oxygen molecules.

13

Carbon dioxide transport: about 70% as bicarbonate ions, 20 to 25% as carbamino-haemoglobin and about 7% dissolved in plasma. The enzyme carbonic anhydrase in RBCs catalyses this reaction.

14

Partial pressures: atmospheric air pO2 is 159 mmHg and pCO2 is 0.3 mmHg; in the alveoli pO2 is 104 mmHg and pCO2 is 40 mmHg. Diffusion always follows the pressure gradient.

15

Respiration is controlled by the respiratory rhythm centre in the medulla oblongata, moderated by the pneumotaxic centre in the pons. The chemosensitive area responds to carbon dioxide and hydrogen ions, not directly to oxygen.

📖 Detailed Notes — English

1. Why we breathe

Every living cell needs a continuous supply of oxygen to oxidise glucose and release energy as ATP, and it must get rid of the carbon dioxide produced. Breathing, or pulmonary ventilation, is the physical movement of air into and out of the lungs. It must be distinguished from cellular respiration, which is the chemical oxidation of food inside mitochondria. Different animals use different respiratory organs: earthworms breathe through moist skin, aquatic insects and prawns use gills, insects use a tracheal system, and frogs use skin, the buccal cavity and lungs. Humans, like all mammals, use a pair of lungs assisted by a muscular diaphragm.

2. The conducting and exchange parts

The respiratory tract is divided into a conducting part and a respiratory or exchange part. The conducting part begins at the external nostrils and runs through the nasal chamber, pharynx, larynx, trachea, primary bronchi and the branching bronchioles up to the terminal bronchioles. Its job is to warm the incoming air to body temperature, moisten it and filter out dust using mucus and ciliated epithelium. No gas exchange happens here, which is why it is also called the anatomical dead space. The exchange part consists of the alveolar ducts, alveolar sacs and the alveoli themselves. The alveolar wall is a single layer of squamous epithelium lying directly against the capillary endothelium, so oxygen crosses a barrier less than one micrometre thick. Type II alveolar cells secrete surfactant, a lipoprotein that lowers surface tension and stops the alveoli from collapsing at the end of expiration — the reason premature babies deficient in surfactant develop respiratory distress syndrome.

3. Mechanism of breathing

Breathing is driven purely by pressure differences, as explained by Boyle’s law. During inspiration the dome-shaped diaphragm contracts and flattens, increasing the vertical height of the thoracic cavity, while the external intercostal muscles pull the ribs and sternum upward and outward. The enlarged thoracic cavity drags the lungs with it through the pleural attachment, so intra-pulmonary pressure drops about 1 to 2 mmHg below atmospheric pressure and air flows in. Normal expiration needs no muscular effort: the diaphragm and intercostals simply relax, the elastic lungs recoil, intra-pulmonary pressure rises above atmospheric pressure and air is pushed out. Forced expiration recruits the internal intercostal muscles and the abdominal muscles. A spirometer records these movements, and the volume of air moved per minute — tidal volume multiplied by breathing rate — gives pulmonary ventilation, normally 6 to 8 litres per minute.

4. Respiratory volumes and capacities

Tidal volume is the 500 mL exchanged in a quiet breath. Inspiratory reserve volume, the extra air taken in by forced inspiration, is 2500 to 3000 mL, and expiratory reserve volume, the extra air pushed out by forced expiration, is 1000 to 1100 mL. Residual volume, 1100 to 1200 mL, always remains in the lungs. Capacities are sums of these volumes: inspiratory capacity is TV plus IRV, functional residual capacity is ERV plus RV, vital capacity is IRV plus TV plus ERV at roughly 4000 to 4800 mL, and total lung capacity is vital capacity plus residual volume at about 5800 to 6000 mL. Vital capacity is the single most useful clinical index of lung health and is higher in athletes and lower in smokers and in restrictive lung disease.

5. Transport of oxygen and carbon dioxide

Gases move entirely by diffusion along partial pressure gradients. Atmospheric air carries oxygen at 159 mmHg and carbon dioxide at only 0.3 mmHg; in the alveoli the figures are 104 mmHg and 40 mmHg. Roughly 97 per cent of oxygen travels bound to haemoglobin as oxyhaemoglobin, each haemoglobin molecule carrying four oxygen molecules, and only 3 per cent dissolves in plasma. Carbon dioxide is carried mainly as bicarbonate ions, about 70 per cent, with 20 to 25 per cent as carbamino-haemoglobin and 7 per cent dissolved in plasma; the enzyme carbonic anhydrase inside red blood cells speeds the interconversion enormously. Every 100 mL of oxygenated blood delivers about 5 mL of oxygen to the tissues, and every 100 mL of deoxygenated blood carries about 4 mL of carbon dioxide to the alveoli. The oxygen dissociation curve is sigmoid; a rise in carbon dioxide, hydrogen ions or temperature shifts it to the right and unloads more oxygen in active tissue, which is the Bohr effect.

6. Regulation and disorders

The respiratory rhythm centre in the medulla oblongata sets the basic pattern; the pneumotaxic centre in the pons can shorten inspiration and raise the rate; and the chemosensitive area beside the rhythm centre responds to carbon dioxide and hydrogen ions rather than directly to oxygen, which is why breath-holding is broken by rising carbon dioxide. Asthma is allergic inflammation and spasm of the bronchioles causing wheezing. Emphysema, caused chiefly by cigarette smoking, destroys alveolar walls and reduces the respiratory surface. Occupational disorders from long dust exposure include silicosis in stone and quarry work, asbestosis, byssinosis from cotton dust and anthracosis or black lung from coal dust. Hypoxia is inadequate oxygen reaching the tissues and causes mountain sickness at high altitude, where low atmospheric pressure, not low oxygen percentage, is the problem.

(Bohr effect).

✅ Answer Key with Explanations  |

Q1 → Option 3 — 97%
Haemoglobin in RBCs carries about 97% of oxygen; only about 3% dissolves in plasma.

Q2 → Option 4 — Residual Volume
Residual volume can never be forced out of the lungs, so a spirometer cannot record it.

Q3 → Option 2 — Medulla oblongata
The medulla houses the respiratory rhythm centre; the pons only moderates it through the pneumotaxic centre.

Q4 → Option 3 — Cotton dust
Byssinosis is the brown lung disease of textile and cotton mill workers.

Q5 → Option 2 — IRV + TV + ERV
Vital capacity excludes residual volume; adding RV gives Total Lung Capacity.

💡 Memory Tips & Mnemonics  |

1. Air pathway — “No Need Pushing Little Toy Bikes Backward Always”

Nostrils → Nasal chamber → Pharynx → Larynx → Trachea → Bronchi → Bronchioles → Alveoli.

2. Gas transport percentages — “Oxygen 97, Carbon 70”

Remember the two headline figures as a pair: Oxygen = 97% as oxyhaemoglobin; Carbon dioxide = 70% as bicarbonate. The leftovers follow the same order: O2 has 3% in plasma, CO2 has 7% in plasma.

3. Vital Capacity — “I Till E, no R”

Vital Capacity = IRV + TV + ERV, and the Residual volume is left out. Add R back and you get Total Lung Capacity. VC about 4600 mL, TLC about 5800 mL — a difference of roughly 1200 mL, which is exactly RV.

4. Occupational lung diseases — “SABA”

Silicosis (stone and quarry dust) · Asbestosis (asbestos) · Byssinosis (cotton, brown lung) · Anthracosis (coal, black lung).

5. Lobes of the lungs — “Right has 3, Left has 2 because the heart leans left”

The heart sits slightly to the left, so the left lung sacrifices one lobe. Right = 3 lobes, Left = 2 lobes.
